Repeat that whole mess four times for each verse of the 
song.  Listen carefully, and there's some pretty random
variation (especially before the second chorus)
going on too, so you can add or delete what you want. 

Now these next chords sound SO much better on guitar, but if you
must, you can play them like this:

Play EACH of these four chords FOUR times for the "What have I become?"
part of the song. Play that whole bit (with the quadruple repetition)
FOUR times, then play the chorus. These chords SEEM to play throughout
the chorus, but the last one (D) doesn't sound right. Tell me if you 
can figure it out.

For simplicity, any spaces should be counted as a rest.

The ending bit before the loud crash seems to be the four chords again
but REALLY distorted. Try it. NOW.. those crash chords I have NO idea 
how to transcribe. They're already pretty freaky on guitar (you gotta 
tune to B and I'm not really up to re-tuning my guitar againa and 
again) so we'll never know I suppose :)
